An Annual Wellness Visit is an office visit that allows us to create a personalized prevention plan. This plan is designed to help prevent disease and disability based on your current health and risk factors. As part of this visit, we will ask you to fill out a questionnaire, called a “Health Risk Assessment.” Answering these questions can help you and your provider develop a personalized prevention plan to help you stay healthy and get the most out of your visit. We may also perform a cognitive impairment assessment to look for signs of Alzheimer's disease or dementia and check for depression and other mood disorders.
If necessary, your doctor may suggest the ordering of other tests, depending on your general health and medical history. While the initial screening visit is free for Medicaid members, these additional tests and services may be subject to a Part B deductible Copay if any of the additional tests and services are not covered under your Medicare preventive benefits. Your doctor will tell you at the time of your visit if he or she is recommending additional testing and what the cost of those additional tests will be to you.
